How to Report Sick or Dead Birds:

  • Poultry Disease Reporting Form: This form should only be used to report illness or death in domestic birds (chickens, turkeys, gamebirds, domestic ducks, etc.). You may submit the form by clicking here OR call the Division of Animal Health at 617-626-1795. Please note that duplicate reporting slows down processing. Division of Animal Health Staff will contact you for more information as needed.
  • Report observations of dead WILD birds: MassWildlife is collecting information on wild birds (gulls, terns, cormorants, sea ducks, etc.) to monitor possible outbreaks of Avian Influenza. They are particularly interested in sick/dead seabirds washing up along coastal areas.
